Achieving a defined six-pack is often seen as the peak of physical fitness, but for women, the relationship between visible abs and overall health is more complex than it appears. According to UK-based surgeon and health educator Dr Karan Rajan, women can develop six-pack abs without necessarily harming their health — but it depends heavily on how that physique is achieved.

Dr Rajan explains that the female body requires a certain level of body fat to maintain proper hormonal balance. Unlike men, women rely on body fat as a key source of estrogen production. When body fat levels drop too low, the brain may reduce or stop sending signals to the ovaries, which can interfere with ovulation and lead to missed or irregular periods. This condition is medically known as Hypothalamic amenorrhea.

Research suggests that for most women, visible abdominal muscles typically appear when body fat falls within the range of approximately 14 to 20 percent. However, as this level gets closer to the essential fat threshold, the risk of hormonal disruption increases. Dr Rajan highlights that losing a menstrual cycle is not caused by abs themselves, but by extreme conditions often used to achieve them — such as severe calorie restriction, excessive exercise, or chronic stress.

Studies involving female athletes also support this concern. In several sports where low body fat is common, more than half of participants reported menstrual irregularities or loss of periods altogether. Beyond reproductive health, extremely low body fat can also contribute to reduced bone density, mental fatigue, and decreased cognitive performance.

At the same time, Dr Rajan emphasizes that having visible abs does not automatically mean a woman is unhealthy. Body composition varies from person to person. Some women naturally store more fat in areas like the hips and thighs, which may allow them to maintain abdominal definition while preserving hormonal health.

Ultimately, the key factor is energy balance. When the body consistently receives insufficient energy — whether from restrictive dieting or intense training — it prioritizes survival over reproductive function. This energy deficit is often the real reason behind hormonal disturbances, not the presence of a six-pack itself.

The takeaway is clear: achieving a toned physique should not come at the expense of overall well-being. Sustainable nutrition, balanced exercise, and adequate recovery play a far more important role than chasing extremely low body fat levels.
