Carrying Your Bag on One Shoulder? Doctors Warn It Could Lead to Early Shoulder Arthritis

Do you always sling your bag over the same shoulder? It might look stylish or feel convenient, but experts warn this everyday habit could be slowly damaging your spine and shoulders. Orthopaedic specialists say that carrying a one-strap bag on the same side repeatedly can lead to chronic shoulder pain, muscle imbalance, and even early arthritis.

According to Dr. Aditya Sai, Senior Consultant in Orthopaedics, Shoulder Surgery, Arthroscopy & Sports Medicine at Dr L H Hiranandani Hospital, Mumbai, the human body is designed for balance, and uneven weight distribution disrupts that natural alignment. Over time, this imbalance causes stress on the shoulder joint, muscles, and spine, increasing the risk of inflammation, shoulder impingement, and early arthritis.

He explains that “when one shoulder continuously bears the weight, muscles like the upper trapezius and rotator cuff become irritated, leading to microtrauma, fatigue, and inflammation. These repeated small injuries can escalate into more serious conditions, including arthritis in the acromioclavicular (AC) joint and glenohumeral joint.”

The imbalance doesn’t stop at the shoulder. It also affects posture and spinal alignment, sometimes leading to neck stiffness, headaches, and back pain. People with previous shoulder injuries or instability are particularly vulnerable to these problems.

To prevent long-term issues, Dr. Sai recommends simple but effective steps:

  • Choose bags with wide, padded straps to spread the weight evenly.
  • Keep the bag’s load light — ideally not more than 10–15% of your body weight.
  • Alternate shoulders regularly if using a single-strap bag.
  • Include shoulder and posture-strengthening exercises in your routine.

Even though shoulder joints are highly mobile, they are also among the most delicate and prone to wear. Regular attention to posture and balance can protect your musculoskeletal health and help you avoid early signs of shoulder arthritis.
